Request for Proposals: Community & Faith-Based Infant and Maternal Health Support Services

Cradle Cincinnati is now accepting proposals for a new funding opportunity through the Ohio Department of Children and Youth to expand the capacity of community and faith-based organizations to carry out impactful, community-level projects that improve maternal and infant health outcomes.   

Nonprofit or faith-based organizations in Hamilton County that serves pregnant women and families with infants up to one year of age are encouraged to apply. This opportunity is open to ALL community and faith-based organizations — your organization does not have to be a current or past Cradle Cincinnati partner to apply for this funding. We welcome new voices and innovative approaches across Hamilton County. 

Funded projects must address at least one of the following social or physical determinants of health: 

  1. Access to early prenatal healthcare and related services;  

  2. Access to quality education;  

  3. Connection with their community; 

  4. Financial stability and independence; and/or  

  5. Quality of housing, neighborhood, and environment. 

Applications that are selected will have their projects included in a comprehensive, community-wide grant submission coordinated by Cradle Cincinnati. 

The submission deadline for applications is Friday, November 14th, 2025 at 5pm via this Google Form.
